Processes that run on email and spreadsheets.

Many processes that sit outside the enterprise systems run on email, spreadsheets and verbal agreement: approvals, request forms, small tracking lists.

Individually they look small; together they create a real loss of traceability. Who approved what, and which version is current, has to be reconstructed afterwards.
